Hearing Loss and Dementia: Why Hearing Health Is Brain Health

Hearing loss is often treated as an inconvenience of aging — something that makes conversations harder, television louder, or restaurants more frustrating.

But the consequences may reach far beyond the ears.

A growing body of research has found a significant relationship between untreated hearing loss and cognitive decline, including an increased risk of dementia. Hearing loss is now recognized as one of the major modifiable risk factors for dementia, meaning it is a factor that can potentially be addressed rather than simply accepted as an inevitable part of aging.

That makes hearing care about more than improving communication.

It may also be an important part of protecting brain health.

The Connection Between Hearing Loss and Dementia

Researchers have followed older adults for years to better understand the relationship between hearing and cognition.

One long-term Johns Hopkins study found that compared with people with normal hearing, those with mild hearing loss had approximately twice the risk of developing dementia. The risk was about three times higher for those with moderate hearing loss and nearly five times higher among those with severe hearing loss.

This does not mean that hearing loss guarantees that someone will develop dementia.

Dementia is complex. Age, genetics, cardiovascular health, lifestyle, education, social isolation and many other factors can influence risk.

But hearing loss stands out because it is both common and treatable.

The 2024 Lancet Commission identified hearing loss as one of 14 modifiable risk factors associated with dementia. Researchers estimate that addressing modifiable risk factors throughout life could potentially prevent or delay nearly half of dementia cases worldwide.

What Happens to the Brain When You Cannot Hear Clearly?

The ear collects sound, but the brain is responsible for making sense of it.

When hearing becomes less clear, the brain receives less complete auditory information. Instead of effortlessly processing speech, the brain may have to work harder to fill in missing pieces.

A conversation that once required little effort can become a constant exercise in concentration.

You may hear someone speaking but struggle to understand the words. You begin using context, facial expressions and guesswork to determine what was said.

That additional listening effort may place greater demands on cognitive resources that are also involved in attention, memory and thinking.

Researchers are still studying exactly how hearing loss influences dementia risk, but several mechanisms are believed to play a role, including increased cognitive load, changes in brain structure and reduced social engagement. Brain imaging research has also linked hearing loss with faster atrophy in areas involved in sound processing and memory.

When Hearing Becomes Difficult, Connection Can Disappear

There is another important part of this relationship.

People do not experience hearing loss in isolation.

They experience it at the dinner table.

In conversations with a spouse.

At restaurants.

At church.

At family gatherings.

During a phone call with a child or grandchild.

As communication becomes more difficult, people may begin avoiding the situations that expose those difficulties.

A person may stop going to restaurants because following the conversation is exhausting. They may speak less during family gatherings because they cannot keep up. They may decline invitations because constantly asking people to repeat themselves becomes frustrating or embarrassing.

Over time, that withdrawal can lead to greater social isolation.

Social isolation itself is another recognized risk factor for dementia. Johns Hopkins researchers have noted that hearing loss can contribute to isolation by making conversation and social participation more difficult.

Hearing loss therefore has the potential to create a cycle:

You hear less.Listening becomes harder.You participate less.You connect less.And the brain receives less of the stimulation that comes from everyday conversation and social interaction.

Can Treating Hearing Loss Slow Cognitive Decline?

This is where the research becomes particularly important.

The NIH-funded ACHIEVE study was the first large randomized clinical trial designed to investigate whether treating hearing loss could affect cognitive decline.

The study followed 977 adults between the ages of 70 and 84 who had untreated hearing loss but did not have substantial cognitive impairment.

Across the entire study population, hearing treatment did not significantly change the rate of cognitive decline over three years.

However, the results were very different among participants who were already at increased risk for cognitive decline.

In that higher-risk group, hearing intervention was associated with a 48% reduction in the rate of cognitive decline over three years compared with the control group.

Later analysis found an even stronger effect among participants at the highest predicted risk. Among those in the top quarter of cognitive-risk scores, cognitive decline was approximately 58% to 62% slower among participants who received hearing treatment.

These findings do not prove that hearing aids prevent Alzheimer's disease or dementia.

But they provide meaningful evidence that treating hearing loss may help slow cognitive decline in some older adults who are already at increased risk.

That distinction matters.

The appropriate message is not that hearing aids are a cure for dementia.

It is that hearing loss is a health factor we can do something about — and addressing it may have benefits that extend beyond better hearing.

Hearing Loss Is One of the Dementia Risk Factors We Can Address

Some dementia risk factors cannot be changed.

We cannot change our age.

We cannot change the genes we inherited.

But hearing loss is different.

It can be identified.

It can be evaluated.

And in many cases, it can be treated.

That is one reason hearing loss has become such an important subject in dementia-prevention research.

The Lancet Commission has repeatedly identified hearing loss as one of the most significant potentially modifiable dementia risk factors. Earlier estimates attributed roughly 8% of dementia prevalence to midlife hearing loss at a population level, making it one of the largest individual modifiable factors identified in that analysis.

This does not mean that treating hearing loss eliminates dementia risk.

It means that hearing health deserves to be considered part of overall health — including brain health.

It Is Not Just About What Your Ears Are Missing

Hearing connects the brain to the world around us.

Conversation provides stimulation.

Voices carry emotion.

Stories exercise memory.

Questions require attention.

Relationships require communication.

When hearing fades gradually, it can be easy to adjust without recognizing how much information has been lost.

The television gets a little louder.

You begin watching people's faces more closely.

You assume other people are mumbling.

You stop joining conversations when several people are speaking.

Eventually, you may not realize how much effort you are using simply to keep up.

That raises an important question:

What are you no longer hearing that your brain still needs?

The laughter from another room.

A quiet comment from your spouse.

The details of a story at dinner.

The voices around a crowded table.

The everyday sounds and conversations that keep you engaged with the people and environment around you.

Hearing care is not simply about making sounds louder.

It is about keeping communication accessible and helping the brain remain connected to the world it is designed to process.

Do Not Wait Until Hearing Loss Becomes Severe

Hearing loss is often gradual, which means the person experiencing it may be the last one to recognize how significant it has become.

If you frequently ask people to repeat themselves, struggle to understand speech in background noise, increase the television volume, or feel mentally exhausted after conversations, it may be time to have your hearing evaluated.

A hearing evaluation can establish a baseline, identify the degree of hearing loss and help determine whether treatment may be appropriate.

There is no guarantee that treating hearing loss will prevent dementia.

But there is increasingly strong evidence that hearing health and brain health are connected — and that untreated hearing loss should not simply be ignored.

Your hearing is not separate from your brain.

The sounds, conversations and connections you experience every day are information your brain uses constantly.

Protecting access to that information may be one of the most important reasons to take hearing loss seriously.
